Research activities. Brief description of major directionsCENTRAL SCIENTIFIC-RESEARCH LABORATORY OF NATIONAL MEDICAL ACADEMY OF POST-GRADUATE EDUCATION named after P.L. SHUPYK
The Central Scientific-Research Laboratory (CSRL) is an independent department of National Medical Academy of Post-Graduate Education named after P.L. Shupyk that submits to the Rector of the Academy and Vice-Rector for science.
HISTORY The CSRL was founded in 1972 according to the Decree of the State Committee for Science and Technology at the Council of Ministers of the USSR and Order of the Ministry of public health of Ukraine of the USSR, under the leadership of prof. V.T. Antonenko. While carrying out scientific studies the CSRL collaborate with departments of the Academy, research institutes of the NAMS of Ukraine and the NAS of Ukraine as well as with research divisions of various universities. Apart from scientific activity the stuff of the CSRL provide methodological and consultative assistance for applicants for a scientific degree and PhD students of the Academy and other educational institutions in planning and carrying out their theses.
STAFF: The Head of the CSRL is K.M.Igrunova, M.D., DSi, since 1998. Under her charge, there was introduced longstanding fundamental research based on the CSRL. The staff include highly-qualified specialists: 2 doctors of medical sciences (DSi in medicine), one being a professor, 3 candidates medical sciences (PhD in medicine), 6 candidates of biological sciences (PhD in biology), one being an associated professor, and 3 senior research associates. All experts have been upgrading their vocational quality.
AREAS OF ACTIVITY: Currently the CSRL is carrying out fundamental and applying studies up to the latest world medicine standards: · Study of regulatory implication of the immune system; · Study of cellular reserve apoptosis mechanisms (baseline) of the body; · Study of genetic determinants of pathology development; · Study of infectious agents and cytokines implication to cardiovascular pathologies; · Study of stress reaction pathogenesis; · Determination of epigenetic regulation involvement in pathology development; · Implication of all mentioned factors to critical conditions and chronic diseases rise.
Based on fundamental studies applied issues are solved to use in health care practice. For example, the CSRL proposed an original diagnostic algorithm for identification of risk groups in cardiology and oncology practice by index of induction of cells apoptosis. New methods of individual selection of pharmaceutical drugs in vitro for patients’ treatment optimization were suggested. Test methods for risk determination and protection against stress pathogenetically-targeted pathology are being developed. Based on the CSRL planning and coordination of the CSRL groups and departments co-executors scientific activity, collecting, statistical processing and analyzing of findings, bibliography selecting and research activity reporting are performed.
SCIENTIFIC ACTIVITY There were accomplished 8 scientific investigations at the Central Scientific Research Laboratory within 2008-2010 financing by Government of Ukrain. The “Role of genetic determinants and immune mechanisms in endotoxicosis progress as a basis for forming metabolic disorders to develop correction methods” fundamental scientific investigation that has purpose investment was being carried out from 2008 to 2010. The following Academy departments took part in this scientific investigation: Medicine of Emergency Conditions; Medical Genetics; Virology; Pathologic and Topographic Anatomy; Gastroenterology and Dietology; Cardiology and Functional Diagnoses; Therapy and Geriatrics; Oncology. In the Clinics there have been studied immune and biochemical markers of proper patent methodology- index of induction of apoptosis in 368 patients with arterial hypertension and metabolic syndrome, ischemic heart disease and type ІІ diabetes combined, hepatitis. It has also been carried out 10000 investigations according to different rates. On the results of the scientific investigation there have been conducted 2 scientific and practical conferences, published 97 scientific papers, obtained 1 patent, 2 methodological guidelines, 3 information letters. “Epigenetic, genetic, biochemical and immune factors for the regulation of mitoses, postmiotic hypertrophy, apoptosis balance in critical conditions pathology progress included sudden cardiac death” budget fundamental scientific investigation that have purpose investment (on NAS of Ukraine, NAMS of Ukraine, Ministry of public health of Ukraine leading specialists reviews) has been being carried out since 2011. Departments of National Medical Academy of Postgraduate Education named after P.L. Shupyk that are participating in this scientific investigation as follows: 1. Emergency Medicine; 2. Heart and Main Vessels Surgery; 3. Medical and Laboratory Genetics; 4. Hospice and Palliative Medicine; 5. Therapy and Rheumatology; 6. Diabetology; 7. Oncology; 8. Ophthalmology; 9. Endocrinology; 10. Cardiology and Functional Diagnoses; 11. Therapy and Pediatrics; 12. Virology; 13. Pathologic and Topographic Anatomy. The departments’ staff members are taking active part in the work by selecting patients, sampling and delivering materials for the investigation. The CSRL staff are performing experimental modeling investigation on animals. The investigation and approbation of suggested methods for modeling sudden cardiac death were conducted. Only successfully tested methods have been selected for further using and patenting. In the Clinics there have been studied immune and biochemical markers in 266 patients. Over 5000 immunological investigations and over 1500 biochemical investigations were conducted using a flow cytometer as well as over 200 preparations to use in morphological investigations were made within 2011. The CSRL staff provide current scientific and methodological consultations about planning and performing dissertations, scientific investigations to applicants and postgraduate students of the Academy and other institutions. Thus, within only 2011 there were provided consultations about planning and performing dissertations to 19 postgraduate students and applicants (PhD and doctorate’s dissertations) from 17 Academy departments as follows: Medical Rehabilitation and Physiotherapy, General and Emergency Surgery, Orthopedic Dentistry, Otorhinolaryngology, Surgery, Children’s and Teenager’s Diseases, Infectious Diseases, Pediatrics №1,Oncology, Anesthesiology, Gastroenterology, Dietology and Endoscopy, Emergency Surgery, Geriatrics and Therapy, Medical Genetics, Neurology №2. Currently B.F. Yakovlev, CSRL correspondent PhD student, is performing a dissertation. Besides, the Central Scientific-Research Laboratory cooperates with: - Scientific Research Institute of Gerontology( Cardiology department), - Scientific Research Institute of Experimental Pathology, Radiology, Oncology named after R. Kavetsky, - Ye.O. Paton Institute of Electric Welding of the NAS of Ukraine, - “ Diagema” Medical Center” ltd. Society, - Poltava Medical University, - National technical university of Ukraine “KPI”, - SE “NSCRM of the NAMS of Ukraine”. In 2010 according to the resolution of the Ministry of public health of Ukraine (№ 135 from 8.06.2000) the CSRL was accredited to carry out clinical investigations on immunology, biochemistry, genetics and morphology i.e. on the results of the attestation which was conducted by Ministry of public health of Ukraine certification verification commission according to approved attestation investigations criteria in the field of the state metrological health care surveillance. The declared attestation during 5 years till 2016 included. In 2011 on the results of scientific and research work there were published 16 scientific works. During the period of the CSRL activity there have been fulfilled 30 Doctoral and 40 Candidate’s dissertations. Laboratory staff have published more than 800 original articles; 87 reports for scientific forums have been made, including 18 abroad, 20 author’s patents for inventions have been received.
EQUIPMENT: PAS Partec flow cytofluorimeter Since 2004 flow cytofluorimeter has been operating in the laboratory. This device has complete software package for cytological analysis, lymphocytes subpopulation analysis (immunophenotyping) and DNA test. The result is imaged as histogram in a real time on a monitor.
BD Medimachine automated system for mechanical tissue disaggregation Since 2011 the laboratory has been equipped with mechanical solid tissue disaggregation device, which allows us to do cytological analyses and biochemical biopsy. The investigations using flow cytometer fulfils by senior staff scientist, PhD in biology D.V. Vatlitsov. Within 2011 he conducted more than 5000 investigations. Platelet Aggregation Analyzer AP 2110 Platelet aggregative properties by turbidimetric method: by continuous measurement of light transmission coefficient with digital indication of results through built-in indicator or computer. The device allows conducting consecutive measurements in one channel with different inducers of platelet aggregation: ADP, collagens, ristomycin, adrenaline. Field of use: · ADP-induced aggregation (diagnosis of hemorrhagic syndrome, pre-thrombotic and thrombotic conditions, congenital thrombocytopathies); · diagnostics of hereditary inborn platelet anomalies (congenital thrombocytopathies: Willebrand's disease, Bernard-Soulier syndrome, etc ); · diagnostics of acquired platelet pathology of hypo- and hyperaggregative nature (diagnostics of leucosis, uraemia, liver diseases and DIC-syndrom); · assessment of antiaggregatory effectiveness of pharmacological medications; · picking up/monitoring of antiaggregant therapy, · assessment of viability of thrombocyte mass during blood transfusion. The studies are conducted by junior researcher Yu.M.Oleshko.
Semi-automatic biochemical analyzer METROLAB 1600DR Analyzer with standard cuvette and flow cell is employed for monitoring optical density on the basis of end point, fixed measure time, kinetics, and absorbance as well as for blood tests. The studies are conducted by PhD in medicine O.V.Lysenko
Labline-100 Automated Biochemical/Immune Enzyme Analizator Up-to-date automated completely open system which doesn’t demand additional staff and using specific reagents to perform complete spectrum of biochemical(enzymes, substrates, lipids, electrolytes, latex and immune turbodimetry of serum proteins), immunoenzyme (in infectious diseases, tumor markers, thyroid gland hormones and antibodies, reproductive and endocrine organs hormones, immunoglobulines, antibodies in case of systemic and autoimmune diseases) analyses to detect oncomarkers, drug preparations and medications, fertility function. Senior staff scientist D.V. Vatlitsov and junior researcher Yu.M. Oleshko conduct investigations.
Spectrophotometer SPh-46 Methods of investigations which can not be done by means of biochemical analyzers are conducted by means of SPh-46 i.e. this device is used for measuring changes of liquid and solid substances transmission spectral coefficients in the area of 190-1100 ηm spectrum. This device detects concentration and speed of optical density changes. In 2011 junior researcher M.G. Anishchuk conducted more than 1500 investigations by means of the device. Microscopes Luminescent microscopes LUMAM-I3 and МBІ-11 allow us to conduct investigations by means of fluorescent marks, phase-contrast analysis, research processes in the dynamics, allow to obtain the image of material in a winning, indirect and combined illumination (quantity, shape, mobility); placed digital photo camera with high-resolution allows us to make digital photos of histological materials (images) with preservation and processing on the computer. Microscopic investigations are conducted by researcher V.D. Pavlyuk and junior researcher M.G. Anishchuk.
Cytogenetic analysis One of the up-to-date methods of genome investigation is cytogenetic and molecular-cytogenetic analysis. Implementation of new diagnosis methods such as fluorescent (in situ) hybridization (FISH), allows us to diagnose 99% of all chromosome pathology and on interphase nucleus in particular. These methods are used for chromosome pathology diagnosis in children with congenital malformations, for detection of the causes of reproductive disorders in married couples and during the prenatal diagnosis. Polymerase chain reaction Polymerase chain reaction method (PCR) opened new possibilities for clinical laboratory diagnosis i.e. detection of genetic mutations and DNA and RNA infectious agents in peripheral blood, serum, plasma, urine, semen, prostate secretion, scrapes and smears (cervical, vaginal and urethral), saliva, washouts, bronchoalveolar lavage, sputum, spinal fluid, articular fluid, tear fluid, breast milk, breast discharge, biopsy material, autopsy material etc. The application of up-to-date technologies allows us to conduct the analysis in a short time i.e. almost during one working day that is very important in the choice of treatment strategy.
